While vaccination is not compulsory in Japan, you may need a vaccine passport in order to travel abroad. This article tells you how to claim your vaccination passport.

What format is the passport and what does it contain?

Currently the passport is available in paper format, but digital format will be released soon.

The passports are offered in English and Japanese and contain your name, date of birth, municipality, type of vaccine, manufacturer and dates the vaccine was administered.

What can you use it for?

You can use it for shortened quarantine restrictions when you re-enter Japan. Or more importantly, you can use it to travel abroad, depending on your country’s requirements.

Who can use it?

The passport is only for people who plan to travel abroad. Those who were vaccinated outside of Japan are not eligible.

How do you apply?

There are three ways to apply.

1) Submit your application by post
Send all required documents plus an addressed return envelope along with an ¥84 stamp. This can take from 1-4 weeks to be approved.

2) Submit your application in person at the ward office
Bring all the required documents to the ward office.

3) Submit your application online
Check the website of your local ward for the link to the online application. You will need scanned copies of all required documents.

Check your local ward’s official website to see if they have a preferred method.

You will need to provide the application form for the passport, proof of vaccination, a copy of your passport which you will use to travel, a copy of your vaccination coupon (or another document proving your address). The vaccine passport application form can be downloaded from the official website of your local ward.
